What Families Should Know About Wound Care at Home

When a loved one comes home from hospital with a wound that needs ongoing care - whether it is a surgical site, a pressure injury, or a leg ulcer - it can feel daunting. You want to help, but you are not a nurse, and the instructions from the hospital may not have been as clear as you needed.

Here is what families should understand about wound care at home, when to call for professional help, and how community nursing can take the worry out of the process.

Not all wounds are the same

Wounds come in many forms, and each type requires a different approach to care. Surgical wounds are typically closed with sutures, staples, or glue and usually heal in a predictable timeframe with proper care. Pressure injuries develop when someone has been immobile for extended periods and can range from mild skin redness to deep tissue damage. Chronic leg ulcers, often related to poor circulation or diabetes, can take weeks or months to heal and require specialist management.

Understanding what type of wound your loved one has will help you know what to watch for and when to seek help.

Warning signs that need attention

While some redness and mild discomfort around a wound is normal, certain signs indicate that something may be wrong and professional assessment is needed. These include increasing pain that is not improving with regular pain relief, spreading redness or warmth around the wound site, unusual or foul-smelling discharge, the wound edges separating or pulling apart, fever or feeling generally unwell, and the wound not showing any signs of improvement over time.

If you notice any of these, contact your loved one's GP or community nurse promptly. Do not wait for the next scheduled visit.

The role of community nurses in wound care

Community nurses - sometimes called district nurses or home visiting nurses - specialise in managing wounds in the home setting. They can assess wound healing progress, clean and redress wounds using appropriate techniques, monitor for signs of infection, adjust treatment plans based on how the wound is responding, liaise with the GP or specialist if the wound is not improving, and educate family members on basic wound care between visits.

What families can do between nursing visits

You do not need clinical training to support good wound healing at home. Keeping the area clean and dry, following the nurse's instructions about when and how to change dressings, ensuring your loved one is eating well and staying hydrated, helping them change position regularly if they have mobility issues, and keeping all follow-up appointments can all make a meaningful difference in healing outcomes.

How to arrange community nursing for wound care

If your loved one has a Home Care Package, wound care nursing can be included as part of their funded services. NDIS participants with wounds related to their disability can access nursing through their plan. For those without funded packages, your GP can arrange community nursing through the local health district, or you can engage a private nursing service.
