How can I apply for HCP?

Connect with My Aged Care for an Assessment
There are many ways to contact My Aged Care.
1. Apply online through www.myagedcare.gov.au.
2. Call My Aged Care on 1800 200 422.
3. Sit down with one of our specialists and they can make a referral for you online or over the phone.

Complete the Assessment at Home.
The assessment is free and face-to-face in the comfort of your own home.
Your local ACAT Assessor will be in contact with you shortly after your application.

Recieve your Home Care Package Approval Letter
Once you’re approved you will be placed on a waiting list until your package is ready to go!
You will also recieve a referral code which we will need in the future!

Recieve your Allocation
Once your package is ready you’ll recieve all relevant details and have 2 months to select a care provider.
